P445 A marathon not a sprint: Establishing a physiotherapist led cystic fibrosis specific Cardio Pulmonary Exercise Testing (CPET) service for Wales

CPET is the gold standard measure of fitness for PwCF in UK. The All Wales Adult CF Centre (AWACFC) established a specific physiotherapy led service to enhance disease monitoring and personalise exercise prescribing. Specific training was undertaken by Physiotherapy clinician teams. protocols safety, risk assessment, infection control, data collection, patient MDT feedback were established. staff members completed questionnaires practice improve testing quality engagement. Pandemic pressures, IT infrastructure challenges learning from test CPETs delayed adoption into routine clinical practice. A total 48 first 5 months at clinic reviews. Barriers included clashes with other investigations, high prevalence virtual reviews, muscle fatigue prior exercise, or exacerbation time test. Positive outcomes increased engagement outpatient breathlessness management, highlighting deconditioning, assisted personal challenges, endurance rowing walking events. Exercise prescribing more individualised using heart rate zones tolerance capacity, focus on limitations beyond lungs. highlighted need knowledge, resources support questions about CPET. focused informal teaching case-based sessions provided. Formal amended increase visibility via electronic record. To establish physiotherapist specialist AWACFC multiple barriers had be overcome. This has enhanced monitoring, service. build current service, ongoing audit, education protocol expansion planned.
